Using Patient Pathways

Clinical pathways help ensure patients receive appropriate and timely care. When developed collaboratively by patients and providers, pathways can be an effective communication tool, improve access to services, and improve patient experiences and outcomes.

Quick-print pathway documents

This page provides easy access to popular 2-4 page printable documents for all pathways. Refer to individual pathways web pages for additional resources including 16-28 page patient education booklets, multimedia resources and links to more web-based information.
